Information collected
The site collects personal information only if you fill in the information request form:
- name and email (required);
- a short reason (optional).
Do not include medical details in the form. To discuss your health, call the office.
Like any website, the host also processes technical data (IP address, browser type, pages requested) to deliver pages and protect the site from abuse. The office does not use it to identify you.
Purpose
Your information is used only to answer your request. It is never sold, rented or used for marketing.
Consent
The form can only be sent if you tick the consent box, which is never pre-ticked. You may withdraw your consent at any time by writing to the person in charge; the office will then delete your request.
Who can access your information
Only office staff who handle requests read form emails. The site relies on these providers:
- Web Hosting Canada: hosting of the website and of the office's email, which delivers the form by email and stores it.
- Cloudflare: protecting the form against automated submissions (Cloudflare Turnstile).
These providers use your information only to provide that service.
Transfers outside Quebec
The website and the office's email are hosted in Canada. Cloudflare Turnstile processes technical data (IP address, browser characteristics) on servers that may be located outside Quebec, including in the United States. Before entrusting information to a provider, the office assesses the privacy factors and ensures, by agreement, that it is adequately protected.
Retention and destruction
The website itself stores no data: no database, no copy of forms, no log of submissions. The email received by the office is deleted once your request is handled, no later than 12 months after receipt.
Security
The site is served over HTTPS only (encrypted connection). Access to the office mailbox is limited to authorized people.
Cookies and tracking technologies
This site uses no tracking cookies, no analytics, no advertising and no technology that identifies, locates or profiles you. Fonts are served from our own servers. Cloudflare Turnstile, used only on the form, blocks bots and is not used for advertising.
Your rights
You may request:
- access to the information the office holds about you;
- correction if it is inaccurate, incomplete or ambiguous;
- deletion, or withdrawal of your consent;
- a copy in a structured, commonly used technological format.
Write to the person in charge at info@bureaudresterez.com. The office replies within 30 days. If you are not satisfied with the answer, you may contact the Commission d'accès à l'information du Québec.
Confidentiality incidents
The office keeps a register of confidentiality incidents. If an incident presents a risk of serious injury, it promptly notifies the Commission d'accès à l'information and the people concerned.
Medical information
Information gathered during a consultation is protected by professional secrecy and kept in your medical record under the obligations of the Collège des médecins du Québec. It does not pass through this website.
